Thank you for your interest in registering for one or more professional development and/or exam certification review Programs (each a “Program,” and, collectively, “Programs”) offered and presented by E2E Health Solutions, LLC, a Texas limited liability company (“E2E”).

 

Your registration and participation in any E2E Program is subject to these Program Registration Terms (these “Registration Terms”), as well as the E2E Terms of Use and the E2E Privacy Policy, each of which are incorporated herein by reference.

  

Registration

 

Registrations are accepted on a first-come, first-served basis.  E2E reserves the right to admit or deny any applicant for any reason (other than in violation of Texas or US law). Incomplete registrations will not be accepted.

 

Payment

 

Payment for all Programs must be received in full at least 72 hours before the start date of the Program, unless online. Payment must be received in full at the time of registration for online, on-demand programs.Program registration fees are nontransferable. Participants registering with a purchase order should follow up with their employer to ensure timely payment.

 

Refund Policy


Except as provided herein, no refunds or Program transfers will be granted.

 

Upon a receipt of a written refund request from a registrant, E2E will provide a full refund of any Program fee actually paid ONLY in the following instances:

 

  1. A registrant completes no more than eight (8) hours or one-half of the Program, whichever is less AND provides a written request for a refund with a “reasonable basis for dissatisfaction” no later than fourteen (14) days after the Program ends.E2E makes the final determination on all refund requests, including whether the registrant’s basis for dissatisfaction is “reasonable”.  
  2. Prior to the start of the Program, a registrant requests a refund, in writing, after receiving notification that the instructor or instructor’s qualifications are different than advertised.
  3. The Program is cancelled by E2E for any reason (e.g. inclement weather or insufficient enrollment).

 

Requests for refunds must be received no later than the earliest to occur of: (i) 14 days from the date the Program was canceled by E2E; or (ii) 14 days from the last date you attended the Program (or accessed, for online Programs).

 

If approved by E2E, refunds will be issued within thirty (30) days of receipt of the refund request.Unless E2E agrees otherwise, refunds will be paid in the same manner as received by E2E, and will be paid to the same party who originally made payment.

  

Professional Development Instructor

 

E2E reserves the right to substitute a Program instructor in the unlikely event that the instructor listed cannot conduct the program. Substitution of the Program instructor is not a cause for refund unless the substitute instructor’s qualifications are less than the advertised instructor’s qualifications.
 

Program Materials and Textbooks

 

You must obtain, at your own expense, all required Program materials and texts.The cost of these materials and texts is not included in the Program fees.

 

Copying of materials, and visual and/or audio recordings of E2E Programs is strictly prohibited without prior written consent from E2E.

 

E2E instructors agree to present materials and contents that are educationally sound for the Program and will provide timely feedback on a participant’s progress. Proper citations and permissions have been used and granted. If an instructor’s proprietary materials are present in the Program, this will be noted on the Program webpage or on the materials, themselves.Otherwise, all copyrights to all materials are owned by E2E or its licensors.

  

Program Cancellation

 

E2E reserves the right to cancel any Program because of low registration or events beyond E2E's control. In the unlikely event of cancellation, registrants will be notified and offered the option to switch to a different Program or receive a full refund.

  

Certificate of Completion

 

E2E provides certificate of completion with hours of professional development for each Program participant’s complete.

  

Participation & Conduct

 

By registering for a Program, you agree to fully participate in each Program meeting and in class discussions, to complete all assignments, and to comply with any Program-specific policies and requirements of any Program instructor (unless such policies and requirements conflict with these Registration Terms). Without limiting the above, you will not engage in any conduct that is disruptive, threatening, harassing, or otherwise disrespectful to the Program instructor or any other Program participant.Violation of this policy will subject you to removal from the Program, and forfeiture of your Program fees.

  

No Guarantees

 

Because people pass and fail exams for a number of reasons, in signing up for a Program, you understand and agree that E2E does not represent or guarantee that you will pass any particular certification examination.

 

Waiver and Consents


Liability Waiver.

You understand and agree that you are participating in the Program voluntarily and at your own risk. You will not hold E2E, nor the owner or operator of any Program locations (each, a “Site”, and, collectively, the “Sites”), nor their respective owners, officers, directors, vendors, contractors, volunteers or employees liable for any negligence or alleged negligence or other fault (not including intentional acts) that results in personal injury, death, or property damage during or in connection with the Program.

 

By participating in the Program, you, for yourself and for your heirs, executors, administrators and assigns, hereby release and forever discharge E2E and all Sites, and their respective owners, officers, directors, vendors, contractors, volunteers and employees from all such claims.

 

This waiver is intended to be comprehensive, and will be construed according to the laws of the State of Texas.


Medical Consent. 

You hereby give your permission for the Sites and E2E representatives to procure all necessary emergency medical help for you, and hereby grant permission to the Sites and E2E representatives to authorize any competent medical person to do all things reasonably necessary to take care of any injury or sickness. There is no health insurance or medical coverage provided by E2E or the Sites, and you accept responsibility for payment of any medical treatment which may be required.


Media.
Your Program participation implies permission to take photographs or videos of Program participants, including you, for E2E’s promotional or publicity purposes (such as brochures, annual reports, webpages, exhibit displays, newspapers, magazines, or television news).


Damage/Disruption.

You understand that you and/or your guests may be barred, excluded or removed from any Program, E2E events and/or Sites for behavior that, in the sole discretion of the Site owners or operators, or the E2E staff, officers or volunteers, is deemed disruptive, dangerous, or offensive.You realize that no refund will be paid in the event you or any of your guests are barred, excluded or removed from one or more Sites.You agree to hold harmless and indemnify the Sites, E2E, and their respective owners, officers, directors, volunteers or employees, from any damage, injury or loss caused to any other person or to any Site caused by any act or omission of you or any guest, child or ward of yours, and agree to pay for any damage, injury or loss associated therewith.
